with a maximum file size of 3MB per PDF.

USA Visa Application Requirements

1. Original recent passport valid for a min. of the last 9 months + Original old passport containing previous US visa
2. 2 sheets of recent color passport photos, size 5cm x 5cm (White Background, clothing color contrasting with the background) * PHOTO SPECIFICATIONS ATTACHED BELOW THESE REQUIREMENTS
3. Print Out Ticket (if any)
4. Hotel Booking (if any)
5. Small form that must be fully completed by the applicant (the data listed in the form will be used by us to fill out the online form on the embassy website)
6. For Child applicants under 14 years old, if both or one of the parents previously held or currently holds a valid USA visa, the Original Child’s Birth Certificate must be attached.
7. Documents MUST BE BROUGHT during the US Embassy interview, as follows:
8. Valid original passport & old passport (containing visa) for reference
9. Original recent color photos with white background size 5cm x 5cm (2 sheets)
10. DS-160 Confirmation (from us)
11. Appointment confirmation (from us)
12. Bank payment slip (from us)
13. EFT sheet (from us)

Supporting Documents

Please prepare and bring originals and photocopies during the appointment, as follows:

1. Original sponsor letter from employing company
2. Original & copy of bank statement for the last 3 months
3. Original & copy of family card
4. Original & copy of ID card (KTP)
5. Original & copy of birth certificate
6. Original & copy of marriage certificate
