IMPORTANT NOTE FOR DOCUMENT SUBMISSION

Please scan and save each required document below into a separate, individual PDF file (unless stated otherwise),

with a maximum file size of 3MB per PDF.

Japan Visa Application Requirements

1. Passport with a minimum validity of the last 7 months + Old Passport
2. 2 sheets of recent color photos, size 4.5cm x 4.5cm (white background & ears visible)
3. Original Sponsor Letter on Company Letterhead
4. Husband’s Consent Letter (if not traveling along, and attach a copy of the Husband’s ID Card/KTP)
5. Copy of Financial Proof for at least the last 3 months (Bank Statement / Savings) + Bank Reference Letter
6. Copy of ID Card/KTP (name on KTP must be the same as on the Passport)
7. Copy of Marriage Certificate
8. Copy of Name Change Certificate (if applicable)
9. Copy of Family Card (KK)
10. Copy of Child’s Birth Certificate + Student ID Card / School Reference Letter
11. Copy of SIUP (if applicable)
12. Print Out Ticket
13. Hotel Booking
14. Invitation Letter from a Japanese Company (for Business purposes) must have a signature and stamp
15. Fill out the Japan visa form

NOTES

For Multiple Entry Japan Visa applications, the applicant must have previously held a Japan visa or holds a UK or USA visa that has been used previously. Additionally, the sponsor letter must state “Request Multiple Entry” on the COMPANY LETTERHEAD.

Electronic Passport (Chip)

1. CHIP Passport with a minimum validity of the last 7 months
2. Old Passport or Copy of ID Card (KTP)
3. Fill out the waiver form
4. Photocopy of Family Card/KK (if family)
5. Statement letter if there is a name difference between Passport and ID Card (KTP)
