Italy Visa Application Requirements
- Passport with a minimum validity of 9 months + Old passport (original & copy)
- 2 sheets of recent color passport photos, size 3.5cm x 4.5cm (Background must be white, Face enlarged 80%)
- Sponsor Letter (In English)
- Husband’s Consent Letter (if not traveling along, and attach a copy of the Husband’s ID Card/KTP)
- Invitation letter from Italy legalized by the local mayor
- Print out ticket
- Hotel confirmation for the countries to be visited
- Travel Insurance must cover Covid (coverage must be above USD 50,000 or EUR 30,000)
- Original Bank Reference Letter (showing the name, account number, final balance)
- Financial proof for at least the last 3 months, updated and legalized by the bank
- Family Card (KK)
- Photocopy of Marriage Certificate
- Photocopy of Birth Certificate
- Photocopy of Student ID Card / School Reference Letter
- Photocopy of ID Card (KTP)
- Photocopy of SIUP (Business License)
- Invitation letter from a company in Italy (for business purposes)
- Itinerary
- Form must be typed (handwriting is not allowed)
NOTES
- For under 18 years of age, both parents must be present during fingerprinting.
- All applicants must attend the appointment, even if they have previously held a Schengen visa.
