Spain Visa Application Requirements
Passport with a minimum validity of 9 months (copy of passport pages 1-3 & 48) + old passport
Passport must not have “see page 4” (name on passport, birth certificate, ID card/KTP, family card/KK must be the same)
2 sheets of recent color passport photos, size 3.5cm x 4.5cm (80% face focus & background must be white)
Sponsor Letter (In English) from Company / both parents
Husband’s Consent Letter + Rp10,000 duty stamp (if not traveling along, and attach a copy of the Husband’s ID Card/KTP)
Photocopy of SIUP (Business License)
Original Bank Reference Letter
Financial proof for the last 3 months (Bank Statement/Savings Account) legalized by the bank
Photocopy of ID Card (KTP)
Photocopy of Family Card (KK)
Photocopy of Marriage Certificate
Photocopy of Child’s Birth Certificate + Student ID Card + School Reference Letter
Photocopy of Name Change Certificate (if applicable)
Print out of International Ticket and Domestic Ticket
Hotel Confirmation
Travel Insurance (coverage must be min. EUR 30,000) 1 month period & cover Covid
City Hall Letter, Invitation Letter, Guarantee to return to Indonesia & inviter’s identity (CNI, Carte de Sejour) * specifically for staying with family
